Taking your medications safely is important for maintaining good health. You may be taking prescription medications or non-prescription medications or both. Our clinical pharmacist is available for patients affiliated with the PEFHT to help you to understand your medications and will help to address any medication issues you may have.
Our pharmacists work collaboratively with:
- Physicians, Nurse Practitioners and Nurses
- Other PEFHT health professionals
- Hospital pharmacists
- Community-based pharmacists

Our clinical pharmacist:
- Provides individual patient assessments to identify, prevent and resolve any drug-related issues.
- Provides general drug information to patients, Primary Care Providers and other interdisciplinary team members taking part in patient care.
- Works collaboratively with QHC-PECM hospital pharmacist and community pharmacists to promote smooth transition when discharged from hospital.

Method of Referral
Patients must be referred to the clinical pharmacist by their Primary Care Provider.
Location and Hours of Pharmacist
The pharmacist is located at the Harbourview Clinic at 35 Bridge Street, Picton, Monday through Thursday, with one day at Sandbanks Medical Centre, every other week.
