PEFHT Rostered Patient Services
Welcome to you and your newborn
As a rostered PEFHT patient in Prince Edward County, you and your well-baby will be scheduled for a series of care visits with your Primary Care Provider or Jenn Howting, Registered Nurse.
Optionally, families can have their Primary Care Provider refer to Jenn, or you can self-refer by calling Jenn directly at 613-813-6512
As a patient rostered with the Prince Edward Family Health Team, here is a list of services and supports available:

Unattached Patient Services - Newborn to 6 Weeks
Welcome to you and your newborn
A referral to our Newborn Clinic can be made from the location of your birthing or via self-referral
To be referred to the clinic, families must meet the following criteria:
- live within Prince Edward County boundaries
- be without a primary care provider OR be with a primary care provider that is located further than a one-hour drive from Prince Edward County
- have a baby that is less than 6 weeks old.
For self-referrals, please call Jenn Howting, RN, at 613-813-6512 to arrange a well-newborn check.
For hospital referrals, the Prince Edward Health Team should contact you within one week post-birth for an appointment. If you have not received a call, please contact Jenn Howting, RN, at 613-813-6512 to arrange a well newborn check.
At the well newborn clinic, your baby will be scheduled for a series of well-baby checks from birth to six weeks of age.
*NOTE: If your baby is unwell, you are advised to seek healthcare from a visit to the PEFHT Community Clinic, a walk-in clinic or hospital emergency room.

Unattached Patient Services - 2 Months to 18 Months
Welcome to you and your baby
The Unattached Well Baby Clinic now accepts self-referrals for babies aged 2 months to 18 months.
To be referred to the clinic, families must meet the following criteria:
- live within Prince Edward County boundaries
- be without a primary care provider
- have a baby that is between 2 months and 18 months old.
Families can self-refer their babies to the clinic by calling PEFHT at 613-476-0400 ext. 239 or a referral fax can be sent to 613-471-1721
This is not a walk-in clinic. It will be providing scheduled appointments for well-baby checkups and immunizations.
The clinics will occur on the 2nd Thursday of every month at the Harbourview Clinic, 35 Bridge Street, Suite 1. Babies will be seen by our Maternal Child nurse, Jenn Howting, and one of our covering physicians.
*NOTE: Families with urgent health concerns can call our PEFHT Community Clinic for an appointment or go to the Emergency Department.

Our next Prenatal Class dates will be:
- 6 pm-8 pm Labour and Delivery – Wednesday, January 15
- 6 pm-8 pm Breastfeeding – Thursday, January 16
- 6 pm-8 pm Bringing Baby Home and the 4th Trimester – Thursday, January 23
Classes will be held at the HUB Early On Child and Family Centre
