PEFHT’s System Navigator is here to assist residents of Prince Edward County with:

  • Education and Empowerment

• Assessing patient needs, goals, and barriers.
• Providing general information and answering system-related questions.
• Helping patients understand available programs, services, and benefits.
• Assisting patients and families with completing forms or applications.
• Supporting transitions between programs, providers, or sectors.

• Coordinating non-medical referrals to internal and external providers.
• Ensuring warm handoffs between services.
• Tracking referral status and following up to prevent gaps.
• Supporting management waitlists and offering interim supports where possible.
• Facilitating case conference or multi-disciplinary discussions.

• Helping patients navigate community, health, social, and government services.
• Identifying the most appropriate services based on eligibility and need.
• Troubleshooting issues when patients encounter service barriers.
• Advocating for patients during system transitions (hospital → primary care, etc).
• Liaising with partner organizations on behalf of patients.
• Liaising with Quinte Health with patient discharges into the community (PEC residents only)

• Educating patients about how to access and use health system resources
• Supporting self-management where appropriate.
• Providing information in a culturally safe and plain-language approach.
• Offering resource guides, brochures, and digital supports.
